Landmark was formed to establish regional hospital referral centers for medically complex patients in need of intensive post-acute care. Landmark established its first regional referral long term acute care hospital in Cape Girardeau, Missouri in 2006 and has steadily grown to seven facilities in four states.
Landmark has successfully established itself as THE regional referral center for medically complex patients in each of its markets, and is well positioned to grow further through the use of its innovative, voice driven electronic health record (ChartPad) and post-acute health information exchange (HIE). ChartPad allows for easy data capture and patient management throughout the entire post-acute continuum. By leveraging this technology, Landmark is well positioned to provide care for and manage the medically complex patient in any post-acute setting thereby reducing readmissions and improving outcomes.
Landmark continues to grow through de novo development and strategic acquisitions in selected markets. Market dominance as a regional referral center for medically complex patients continues to be the driving vision at Landmark. Through the use of data capture and analytics, we strive to place the right patient in the right post-acute setting and care for and monitor that patient throughout the post-acute period.
